§ 6.03. Form of petition.

  • All petition papers circulated for the purpose of initiative or referendum shall be uniform in size and style, and all initiative petition papers shall contain the full text of the proposed ordinance. The signatures to initiative or referendum petitions need not all be appended to one (1) paper, but to each separate petition paper shall be attached a statement of the circulator thereof as provided by this section. Each signer shall sign his or her name in ink or indelible pencil and shall list his or her full residence address. Each petition paper shall contain the names and addresses of five (5) electors who shall be regarded as the petition committee, who shall be the same on each petition paper and shall be responsible for the circulation and filing of the petition. Attached to each separate petition paper shall be an affidavit of its circulator that the circulator alone personally circulated that paper, that it bears a stated number of signatures, that all the signatures appended thereto were made in the presence of the circulator, and that the circulator believes them to be the genuine signatures of the persons whose names they purport to be.
